On-Site Behavioral Health Program in Nursing Facilities

While Behavioral Health Services become less available and turn toward telehealth daily while there is a  continual mandate for Gradual Dose Reductions (GDRs) based on place of setting vs. patients' needs.

 

We are a healthcare service organization that provides on-site behavior health services to Skilled Nursing Facilities (SNFs), Long-Term Care Facilities (LTCs),  Assisted Living Facilities, Memory Care Facilities, Continuing Care Retirement Communities (CCRC), Clinics,  and other locations where elderly patients are in the greatest need of attention and behavioral healthcare. Providing care at a faster response time, coupled with better face-to-face interactions between the patient and provider results in improved patient-centered care.

 

Our Nurse Practitioners and Licensed Clinical Social Workers (LCSWs) become a strategic part of the provider care team in the facility, but work collaboratively with the Medical Director and other physicians in the facility. Our team is in the facility weekly, at a minimum along with being accessible 24/7,to stay on top of acute changes in the condition of any patients and to ensure that those in skilled care receive the best care.  This type of high quality care, along with Patient and Facility focuses, leads to fantastic outcomes and happier patients, families and staff.
